This is a home-based position with field responsibilities, approximately 25% of the time within Jefferson or Kitsap Counties. If you live in or within commutable driving distance to Bremerton, Port Orchard, Port Townsend, or Silverdale, WA, you'll enjoy the flexibility to work remotely as you take on some tough challenges.

You'll be responsible for solving moderately complex problems and conducting moderately complex analyses. You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role.

Proficiency with Microsoft Word and Outlook is required. You'll also need to be able to travel up to 25% within a 60-mile radius from residence to make field visits. A minimum of 1+ years of experience working within the community health setting in a healthcare facility is required, as well as 1+ years of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, or home health.

Experience in managing populations with complex medical or behavioral needs is essential, as well as experience working with Home Care Based Services and/or patients in community and home-based settings. Field-based work experience is also required.

All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy.

The hourly range for this role is $28.95 per hour. Pay is based on several factors, including local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
