Job Summary:

We are seeking a skilled Clinical Pharmacist to provide high-quality patient care and support our healthcare team at Matrix Providers. The ideal candidate will have a Doctor of Pharmacy (Pharm.D.) degree and experience in clinical pharmacy practice. As a Clinical Pharmacist, you will assess patient responses to drug therapy, plan and implement medication regimens, and collaborate with healthcare providers to ensure optimal patient outcomes.

Key Responsibilities:

- Assess patient responses to drug therapy and plan medication regimens
- Order and evaluate laboratory tests to evaluate drug therapy effects and outcomes
- Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states
- Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics

Requirements:

- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
- Current, full, active, and unrestricted license to practice as a pharmacist
- Basic Life Support for Healthcare Providers certification
