Job Title: Clinical Care Manager

We are seeking a highly skilled and compassionate Clinical Care Manager to join our team at Family Service Association - Fall River. As a Clinical Care Manager, you will play a vital role in leading our interdisciplinary team in providing high-quality care to individuals with long-term health care, behavioral health, and social determinants of health needs.

About the Role:

The Clinical Care Manager will be responsible for supervising a team of Care Coordinators and Community Support Workers, ensuring the delivery of all LTSS CP Support Interventions. This role requires a strong understanding of care coordination, bi-lingual capacity, and the ability to work with individuals from diverse backgrounds.

Responsibilities:
  • Supervise Care Coordinators and Community Support Workers on a regular basis, providing guidance and support to ensure the delivery of high-quality care.
  • Develop and implement person-centered care plans that reflect the voice and choice of assigned enrollees.
  • Ensure LTSS CP Supports care plans are endorsed by primary care providers and shared with all members of the enrollee's care team.
  • Support the engagement of assigned enrollees within specified timeframes.
  • Promote achievement of Quality Measures for the LTSS CP Supports.
  • Support the development of Consumer Advisory Boards and provider networks.
  • Have a willingness and ability to work with ACOs, MCOs, state agencies, service providers, and community organizations.
  • Provide guidance to the care coordination team for the development of health and wellness coaching and current listing of community resources.
  • Participate in 24-hour on-call coverage on a rotating basis.
  • Triage referrals and provide referral resources with accurate and timely availability of needed resources.
  • Maintain paperwork requirements for LTSS CP Supports in compliance with established timeframes.
  • Assist in the orientation of new employees and the ongoing professional development of all LTSS CP Supports employees.
  • Provide assistance to the Director in reporting and monitoring program performance.
  • Demonstrate and promote respectful, culturally competent, and linguistically responsive delivery of services.
  • Understand and demonstrate commitment to agency mission, values, and philosophy in performing job responsibilities, including compliance with all ethical and legal requirements of the organization.
  • Respond to reasonable expectations of the Director of Healthcare Transformation and the organization.
Requirements:
  • Minimum of 1 year experience as a supervisor and a minimum of three years of relevant professional experience.
  • Skills necessary to engage and work with individuals from diverse backgrounds.
  • Maintain a nonjudgmental attitude toward families.
  • Willingness to work across the geographic region assigned to the team and demonstrate willingness to work with the target population.
  • Care Coordination, bi-lingual capacity is desired.
Preferred Qualifications:
  • Master's Degree or a RN with behavioral health expertise.
  • Preferred credentials: LCSW, LICSW, LMHC, LMFT, LADC 1, Licensed Psychologist, Licensed Rehabilitation Counselor, or Registered Occupational Therapist.
