We are seeking a highly skilled and compassionate Clinical Care Manager to join our team at Family Service Association - Fall River. As a Clinical Care Manager, you will be responsible for leading an interdisciplinary team in providing long-term support services to children and adults with complex health care needs.
Responsibilities:- Supervise a team of care coordinators and community support workers to ensure the delivery of high-quality services.
- Develop and implement person-centered care plans that reflect the voice and choice of assigned enrollees.
- Collaborate with primary care providers and other members of the care team to ensure seamless care coordination.
- Engage with individuals with complex long-term support and behavioral health needs.
- Support the achievement of quality measures for the LTSS Community Partners program.
- Develop and maintain relationships with community organizations and service providers.
- Participate in on-call coverage and provide guidance to the care coordination team.
- Master's degree or RN with behavioral health expertise.
- Minimum of 1 year experience as a supervisor and 3 years of relevant professional experience.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Bi-lingual capacity is desired.
