We are seeking a skilled and compassionate Registered Nurse to join our team at Family Service Association's Adult Day Health program in Mashpee, MA. As a Nurse Care Coordinator, you will play a vital role in ensuring the health and well-being of our participants.
Key Responsibilities:
- Act as liaison between participants' primary care physicians and our program staff.
- Monitor participants' health status and respond to any acute changes.
- Administer medication and document on medication sheets.
- Supervise program aides and other health care staff members.
- Provide case management services and review participants' personal care plans.
- Assist with ADLs and overall health care needs.
- Complete nursing progress notes and ensure compliance with relevant regulations.
Requirements:
- Licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts.
- At least two years of recent experience in direct care of older adults or adults with disabilities.
- CPR and First Aid certification.
