Position Overview:

The Cardiac Care Nurse Manager is a pivotal role within our healthcare team, focusing on the management and coordination of patient care in the cardiac service line. We are seeking a candidate with substantial leadership experience, ideally at the director level, to enhance our cardiac services.

Key Qualifications:

To be considered for this position, candidates should possess:

  • A minimum of 2-5 years of experience as a Nurse Manager or in a similar leadership role.
  • Registered Nurse (RN) license valid in New York State.
  • PRI Certification (to be obtained within 3 months of employment).
  • Strong preference for candidates with screen certification and care management certification (e.g., CMC, CCM).
  • At least 3 years of clinical experience in Emergency Department, Critical Care, or Medical/Surgical settings.
  • Prior experience in case management is highly desirable.
  • Familiarity with healthcare financial, regulatory, and payer issues.
  • Knowledge of state, local, and federal healthcare programs.
  • Experience with Milliman/InterQual criteria is a plus.
  • Essential computer skills.

Role Responsibilities:

The RN Care Manager will:

  • Conduct comprehensive assessments upon patient admission, adhering to departmental policies.
  • Screen patients for high-risk indicators to ensure appropriate support services are provided to prevent readmission.
  • Perform ongoing reassessments throughout the patient’s hospital stay.
  • Engage in utilization review activities as per departmental guidelines.
  • Facilitate multidisciplinary care coordination within and between hospitals.
  • Oversee transitional planning activities, including patient and caregiver education, aftercare arrangements, and collaboration with commercial payers.
  • Communicate relevant patient information effectively to the healthcare team.
  • Contribute to departmental goals, including reducing length of stay, preventing readmissions, and improving patient satisfaction.
  • Document all activities in accordance with departmental requirements.
  • Participate in performance improvement initiatives as necessary.
