Job Description

We are seeking a skilled NP or PA to join our HouseCalls team in Prineville, OR. As a member of this team, you will provide annual clinical assessments to patients in the comfort of their homes. This role involves conducting in-home evaluations, performing screenings, identifying diagnoses, and communicating findings with the patient's primary care physician.

In this position, you will have the opportunity to focus on preventive care and improve the lives of millions while working collaboratively with our integrated medical teams.

We offer flexible scheduling, allowing you to achieve a work-life balance. No on-call shifts, weekends, or holidays are required.

Required Skills and Qualifications

To be successful in this role, you must have:

  • An active NP or PA license or the ability to obtain one by start date
  • A national certification from ANCC or AANP (for NPs) or NCCPA (for PAs)
  • The ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
  • Reliable transportation for home visit assessments
