About the Job

We are seeking a highly skilled Social Worker to join our team at the Veterans Affairs, Veterans Health Administration. As a Social Worker, you will play a critical role in providing comprehensive care to our nation's veterans.

Job Summary

The Social Worker will work as part of the Patient Aligned Care Team (PACT) to deliver primary care to veteran patients in a longitudinal rather than episodic fashion. The focus will be on prevention, health promotion, coordination, and chronic disease management.

Responsibilities
  • Provide PACT Social Work Services to veterans in assigned regions using various modalities, including face-to-face, virtual, and telephone services.
  • Provide panel coverage as needed and hold a valid driver's license.
  • Maintain effective therapeutic relationships with veterans in the Primary Care Clinic and/or their families.
  • Independently work with veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal, and psychosocial problems.
  • Implement treatment modalities in working with individuals, families, and groups to achieve treatment goals.
  • Facilitate action for community placements through collaboration with veterans and their families, as well as interdisciplinary treatment team members.
  • Provide consultation and education to veterans and their families regarding community resources, VA benefits, and specialty programs.
  • Coordinate with staff at each assigned Community-Based Outpatient Clinic (CBOC) and telehealth coordinators to provide clinical telehealth services.
Requirements
  • Must be a U.S. Citizen.
  • Must be proficient in written and spoken English.
  • Must pass a background/security investigation.
  • Must be able to work independently and as part of a team.
  • Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Must be licensed or certified by a state to independently practice social work at the master's degree level.
Preferred Qualifications
  • Medical or hospital social work experience.
