Key Responsibilities- Coordinate and implement post-discharge plans in collaboration with Case Managers and other healthcare professionals.
- Assist with advocacy and referrals to community resources to support patients and their families.
- Work closely with interdisciplinary teams to develop and implement patient-centered care plans.
- Provide education and support to patients and their families to promote health and well-being.
- Collaborate with healthcare providers to ensure continuity of care and address any barriers to care.
- Bachelor's degree in Social Work or related field.
- 3 years of experience in case management, discharge planning, social work, managed care, or professional counseling.
- Experience in a hospital or healthcare setting preferred.
- Licensure or eligibility for licensure in the state of practice.
