Key Responsibilities- Assess and Address Patient Needs
Conduct thorough assessments of patients' emotional, environmental, and social factors to identify areas of concern and develop comprehensive treatment plans.
Provide Community Resource PlanningAssist patients in accessing community resources, such as home health care services, to meet their long-term care needs.
Update Case RecordsMaintain accurate and up-to-date case records, including progress notes and treatment plans.
Collaborate with Healthcare TeamsWork closely with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination.
Provide Education and SupportOffer education and support to patients and their families on managing chronic conditions, accessing community resources, and navigating the healthcare system.
Requirements- Master's Degree in Social Work
A Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program is required.
Experience in HealthcareAt least one year of experience in a healthcare setting, preferably in a social work or counseling role.
Licensure and CertificationA current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are required. Additionally, licensure as a Licensed Clinical Social Worker (LCSW) from the State Board of Social Work is required in Colorado.
