Key Responsibilities- Conduct initial and ongoing psychosocial assessments to identify patient and family needs
- Develop and implement individualized plans of care, including bereavement support and advanced directives
- Collaborate with the IDT/IDG to ensure seamless care coordination and communication
- Provide education and resources to patients and families on disease processes, coping strategies, and available community resources
- Identify and address potential risk indicators, ensuring timely interventions and referrals
- Master's degree in social work from an accredited program
- One year of social work experience in a healthcare setting
- Current Colorado Social Worker license (LSW or LCSW) or willingness to obtain
- Valid Colorado state driver's license and proof of auto insurance
