Care Management Specialist

We are seeking a skilled Medical Care Navigator to support our patients in accessing necessary healthcare services. As a key member of our care team, you will work collaboratively with clinicians, administrators, and other stakeholders to ensure that patients receive comprehensive and coordinated care.
Your responsibilities will include performing outreach and navigation services, utilizing strength-based patient-centered motivational interviewing techniques, and participating in the development and implementation of care coordination processes.
The ideal candidate will have a strong understanding of community health, discharge planning, and chronic disease management. Experience working as a part of an inter-professional team is also highly desirable.
Salary Range: $70,000 - $90,000 per year
