About the Role

We are seeking a highly skilled and compassionate Serious Illness Care Navigator to join our team at Blodgett Hospital. As a key member of our Advance Care Planning department, you will play a vital role in supporting patients and their families navigate the complexities of chronic illness and end-of-life care.

Key Responsibilities
  • Assess patient needs and develop personalized care plans to meet those needs.
  • Provide education and support to patients, families, and caregivers to ensure seamless care transitions.
  • Collaborate with healthcare teams to ensure comprehensive and coordinated care.
  • Facilitate open dialogue between patients, families, and care providers to ensure patient-centered care.
  • Support patients and families in exploring their values, goals, and preferences for care.

Qualifications
  • Master's Degree in Social Work (MSW) from an accredited program.
  • 3 years of relevant experience supporting individuals with complex medical conditions and social determinants of health.
  • Licensed Master Social Worker (LMSW) in the State of Michigan (or willing to obtain within 6 months).
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross.
