This role includes providing education and support to patients, families, staff and caregivers. Demonstrates expertise, current knowledge in care management of a caseload of patients with high complexity and seeks to improve patient, family and health systems /community outcomes through the application of education concepts/skills and planning for medical crisis and endof life in a management care environment. SICNs will have visits with patients and families while hospitalized, in post-acute (skilled nursing facilities) and completed home visits to manage patient goals and needs for their health care.Knowledge and ability to ask appropriate clinical questions, and illicit patient information to determine appropriate action(s).Improve patient experience by connecting patient and family with appropriate resources, services, and community resourcesthat align with what matters most to them.Priorities include improving paitent and families understanding of health status and eliciting what matters most in the context of their heath.Access to multiple network clinical information systems to gather and verify patient demographic and clinical information.Collaborate with all care providers to assist in the provision of quality, cost effective patient care.Provide individualized support, education and information, helping to make care seamless, continuous and comprehensive.Serve as a liaison between the patient and their family and the various hospitals, physicians offices, community programs and post-acute facilities with whom they interact.Responsible for patients within the inpatient, outpatient, and post acute (skilled nursing facilities).How we will care for you, while you care for our patientsComprehensive benefits package to meet your financial, health, and work/life balance goals.
