The Serious Illness Care Navigator will evaluate each patient's unique needs and create a personalized care plan.
This position involves providing education and support to patients, families, staff, and caregivers, demonstrating expertise in managing a caseload of patients with complex medical conditions.
Patient EngagementSICNs will conduct visits with patients and families in various settings, including during hospitalization, in skilled nursing facilities, and through home visits, to effectively manage healthcare goals and needs.

Collaboration and Support- Facilitate understanding of health status and priorities among patients and families.
