We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home-Based Medical Care Provider in Hartford and Litchfield County, CT. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities- Conduct comprehensive assessments of patients' medical and social needs, developing and implementing individualized care plans to address their unique needs and goals.
- Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication.
- Prescribe and manage medications, order diagnostic tests, and provide medical interventions as needed to promote optimal patient outcomes.
- Participate in ongoing education and training to stay current on best practices, research, and clinical guidelines in home-based medical care.
- Contribute to quality improvement initiatives and participate in ongoing meetings to discuss patient care and clinical excellence.
- Completion of a Nurse Practitioner or Physician Assistant accredited program.
- Current state RN/NP or PA license.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA).
- Access to reliable transportation.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care.
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
