POSITION SUMMARY

This role is essential for the assessment, planning, execution, and oversight of Physical Therapy interventions in alignment with physician referrals, state and national standards, regulatory guidelines, and established departmental protocols. The individual in this position exhibits the clinical expertise and knowledge necessary to deliver age-appropriate treatment for the patients served.

CORE FUNCTIONS
1. Conducts thorough evaluations of patients' functional capabilities and restrictions. Determines the necessity for intervention and formulates a comprehensive care plan with measurable treatment objectives. Completes detailed assessments within the designated timeframes set by the department. Identifies and prioritizes critical limiting factors, setting functional and patient-centered treatment goals in collaboration with patients and their families. Initiates discharge planning and suggests modifications to referring orders as needed.

2. Executes tailored treatment plans considering the patient's physical, social, spiritual, cultural, educational, and age-specific requirements. Provides skilled interventions safely and assesses the ongoing effectiveness of treatment, adjusting the care plan as necessary. Engages patients and their families in the treatment process and decision-making.

3. Maintains documentation in accordance with professional practice standards mandated by regulatory bodies. Ensures all records are clear and completed within the department's established timelines. Documentation should reflect evidence-based and individualized treatment strategies, showcasing skilled interventions and comprehensive evaluations of patient progress.

4. Effectively communicates relevant patient information to facilitate optimal continuity of care. Shares evaluation and treatment outcomes with physicians, nursing staff, patients, families, and interdisciplinary teams in a timely and clear manner. Takes responsibility for reaching out to physicians for routine or complex problem-solving to ensure the best patient outcomes.

5. Demonstrates flexibility in managing assignments related to caseload, scheduling, and staffing variations. Proactively assists in other patient care areas and takes responsibility for all patient care needs.

6. Oversees the appropriate supervision and delegation of tasks to ensure the safe delivery of quality, cost-effective patient care. Compliance with departmental policies and regulatory standards is essential when supervising and delegating to PTAs, ATCs, Therapy Technicians, and students. Participates in the orientation and training of new staff, ensuring their competency before delegating tasks.

7. Engages in departmental operational activities. Attends required meetings and follows up to gather relevant information. Actively participates in departmental and facility process improvements while meeting the productivity benchmarks established by the department.

8. This position requires interaction with various staff levels across multiple departments, physicians, patients, families, and external contacts, including employees from other healthcare institutions and community agencies regarding patient care. Additionally, communication with physician offices is necessary to report, request, or clarify information.

MINIMUM QUALIFICATIONS
Must have graduated from an APTA accredited Physical Therapy program or its equivalent.

Must hold a current Physical Therapy license in the state of practice. BLS certification is required.

Candidates should demonstrate their clinical competence, experience, training, and education relevant to the role throughout the interview process.

Additional related education and/or experience is preferred.
