About the Role
At CenterWell Home Health, we are dedicated to delivering personalized care that puts patients at the center of their healthcare experience. As a skilled Home Health Registered Nurse, you will play a vital role in helping our patients regain their independence and live healthier lives.
Key Responsibilities
- Provide comprehensive care to patients in the comfort of their own homes, including admission, case management, and follow-up visits.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans that meet each patient's unique needs.
- Conduct thorough assessments and evaluations to identify areas for improvement and develop strategies for achieving optimal outcomes.
- Supervise and educate home health aides and personal care workers to ensure seamless delivery of care services.
- Maintain accurate and detailed records of patient interactions, treatments, and progress, ensuring compliance with regulatory requirements.
- Communicate effectively with patients, families, and healthcare teams to provide timely updates and address any concerns or questions.
- Participate in ongoing professional development to stay current on best practices and industry advancements.
Requirements and Qualifications
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related field
- Strong clinical skills, including med-surg, ICU, ER, acute care experience
- Current and unrestricted RN licensure
- CPR certification
- Excellent communication, organizational, and time management skills
Compensation and Benefits
We offer a competitive salary range of $69,800 - $96,200 per year, commensurate with experience. Additionally, you'll enjoy a comprehensive benefits package, including paid time off, 401(k) retirement savings plan, employee assistance program, business travel insurance, and accident insurance.
