Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at KC CARE Health Center. As a Care Coordinator, you will play a critical role in providing strengths-based Case Management services to individuals living with HIV. Your primary responsibility will be to facilitate and monitor client service acquisition and progress, with the ultimate goal of successful engagement in comprehensive HIV care.

Key Responsibilities
  • Conduct comprehensive assessments and create individualized service plans for clients
  • Evaluate client eligibility for government-funded programs and provide referrals and follow-up as necessary
  • Make referrals to community resources and services based on client need and eligibility, and coordinate follow-up care and decisions affecting the client
  • Provide education to clients on HIV, medication and medical appointment adherence, viral load and CD4 lab results, and risk reduction strategies
  • Collaborate with internal and external partners regarding client care and goals
Requirements
  • Bachelor's degree in Social Work, R.N., B.S.N., or 2 years' experience in Case Management type services
  • Less than 3 years' post-graduate experience in Case Management type services or Master's degree in Social Work, Nursing, or related field
  • Demonstrated experience serving a diverse population
