Job Description:
We are seeking a highly skilled and compassionate Staff Nurse to join our PCM department. As a Staff Nurse, you will play a critical role in providing essential services to People with Disabilities in their homes. Your expertise will be used to carry out initial evaluations and annual re-evaluations, identifying available resources and supports needed by consumers.

Responsibilities:
  1. Conduct home visits to assess consumer needs.
  2. Develop and implement individualized care plans.
  3. Collaborate with healthcare professionals and community resources to ensure comprehensive care.

Requirements:
  1. Registered Nurse or Licensed Practical Nurse degree.
  2. At least 1 year of experience in community-based health care.
  3. Knowledge of disability support services and community resources.
  4. Excellent communication and interpersonal skills.
  5. Ability to work independently and as part of a team.
