Community Support Specialist

**Job Summary:**
We are seeking a highly skilled and compassionate Community Habilitation Provider to join our team at Summit Educational Resources. As a key member of our staff, you will play a vital role in providing person-centered support to individuals with developmental disabilities.
**About the Role:**

As a Community Habilitation Provider, you will be responsible for ensuring that individuals receive the highest quality care and support, enabling them to live fulfilling and independent lives.
**Key Responsibilities:**
• Provide individualized support to clients, focusing on their unique needs and goals.
• Develop and implement personalized plans to promote community integration and participation.
• Train clients in essential life skills, such as personal hygiene, budgeting, and meal preparation.
• Document services and progress notes accurately and contemporaneously.
• Collaborate with colleagues and supervisors to achieve minimum caseload requirements.
• Participate in reviews and updates of the Community Habilitation Staff Action Plan to ensure continuous improvement.
**Requirements:**
To be successful in this role, you will need:
• A High School Diploma and at least one year of combined work, volunteer, or internship experience working with individuals with developmental disabilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong problem-solving and crisis management skills.
• Passion for supporting individuals with developmental disabilities.
