About This Role

We are seeking a skilled Clinical Care Coordinator to join our team at Commonwealth Care Alliance. As a key member of our Care Planning Unit, you will play a vital role in developing and ensuring adherence to high-quality, member-centric care plans.

Key Responsibilities
  • Conduct timely, in-person assessments to gather information about members' health and care needs.
  • Develop and document person-centered care plans using data collected during the assessment process.
  • Collaborate with other members of the Care Planning Unit and staff in other departments to support timely interventions designed to improve outcomes for CCA members.
  • Communicate effectively with members, families, and healthcare providers to ensure seamless care coordination.
  • Participate in assessing and improving quality within the scope of responsibilities and throughout the organization.
Requirements
  • Associate's Degree in Nursing required, Bachelor's Degree in Nursing preferred.
  • RN with active registered nursing license in Commonwealth of Massachusetts, in good standing required.
  • 2+ years of experience in clinical settings, with a focus on care management and coordination.
  • Strong skills in clinical decision-making, care delivery, and communication.
  • Ability to work independently and as part of a team.
