Join Our Team as a Health/Human Service Case Manager II

We are seeking a highly skilled and compassionate Health/Human Service Case Manager II to join our team at the Delaware Department of Health and Social Services. As a key member of our team, you will play a vital role in improving the quality of life of Delaware citizens by promoting health and well-being, fostering self-sufficiency, and protecting vulnerable populations.

About the Role

The incumbent in this position will determine initial and continued ongoing eligibility for all needs-based benefits issued by the Division, which includes Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children's Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). You will work closely with TANF and SNAP Employment and Training vendors to move clients to self-sufficiency.

Key Responsibilities

  • Interview clients, families, or professional sources to determine their needs for various health and human services.
  • Develop comprehensive cost-effective service plans utilizing in-house sources, referrals, contracted providers, or institutions.
  • Conduct ongoing monitoring and assessment of service delivery for optimum quality and efficiency.
  • Maintain comprehensive case records, summaries, statistical and narrative reports, and complete required forms.
  • Establish effective working relationships with clients, family, community, and professional sources.
  • Provide guidance and relevant information to connect clients with needed services.
  • Communicate and interact effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.

Requirements

Applicants must have education, training, and/or experience demonstrating competence in each of the following areas:

  • Six months' experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  • Six months' experience in making recommendations as part of a client's service plan, such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  • Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  • Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data.
  • Six months' experience in narrative report writing.
