Job Description

Overview

AIDS Healthcare Foundation is seeking a dedicated Medical Care Manager to join our team.

The ideal candidate will have a strong background in case management and a passion for working with vulnerable populations.

The Medical Care Manager will be responsible for providing education and assistance to clients, completing Needs Assessments, developing Action/Care Plans, and providing ongoing education to patients.

The successful candidate will be a passionate advocate for our top initiatives and will work closely with our healthcare center team to ensure that every client receives the support they require.

Responsibilities:

  • Provide education and assistance to clients
  • Complete Needs Assessments and develop Action/Care Plans
  • Provide ongoing education to patients
  • Work closely with our healthcare center team to ensure client support

Requirements:

  • Bachelor's or master's degree in Social Science, Social Work, or Human Services
  • Minimum of 1 year of Case Management experience
  • Knowledge of Ryan White Services is preferable
