Position Overview:
A nonprofit organization dedicated to community services is seeking a qualified Case Planner to join their Foster Care initiative.


About the Role:
Start Date: ASAP
Assignment Length: Ongoing (with potential for contract to hire)
Schedule: Monday to Friday, 9am to 5pm
Work Environment: Hybrid (2 days in the field & 3 days onsite)


Target Population:
Children in Foster Care (Ages 0-21)


Responsibilities:
The Case Planner will be tasked with the following duties:
1. Collaborate with Foster and Biological Parents to establish a child’s permanency plan.
2. Provide comprehensive support and case management services to children in foster and adoptive settings.
3. Prepare and distribute referral packages for essential services to relevant community resources.
4. Ensure the implementation of service plans for all children in care.
5. Schedule and oversee visitations between children and their families.
6. Participate in Family Team Conferences (FTC) and attend court hearings, hospital visits, and other necessary appointments.
7. Follow up on action items from FTC meetings.
8. Undertake additional responsibilities as required.


Qualifications:
- Minimum of 2 years of relevant experience.
- Bachelor’s Degree in Social Work.
- Familiarity with working with youth in foster care and families in crisis.
- Proficient in Microsoft Office.
- Knowledge of OCFS Connections.
- Strong assessment and documentation abilities.
- Excellent interpersonal and communication skills (both written and verbal).
- Exceptional attention to detail and organizational skills.


Desired Skills:
- Bilingual proficiency in English and Spanish is preferred.
