Job Description

Position Overview:

The Case Planner for the SILP Program plays a crucial role in delivering comprehensive case management services for a group of young individuals enrolled in the program. This position involves providing essential services, resources, and counseling, while being an integral part of the SILP team dedicated to enhancing the outcomes for the youth.

The Case Planner will be responsible for offering support during the residents' time in care and will also extend aftercare services following their discharge. Additionally, the Case Planner is tasked with ensuring that all necessary documentation required by relevant authorities is completed accurately and in a timely manner.

Position Qualifications:

  • A Bachelor’s degree in Social Work or a related field from an accredited institution, with a preference for candidates holding a Master’s degree in Social Work.
  • A minimum of three years of experience in foster care as a case planner, along with familiarity with the regulations and systems of ACS, NYS/OCFS, and CONNECTIONS.
  • Proficient in preparing essential documentation such as FASPs, Permanency Hearing Reports, Incident Reports, Service Plans, Treatment Plans, and Progress Notes.
  • Comprehension of the importance of precise and timely documentation in service delivery and case management practices, alongside a solid understanding of child and adolescent development.
  • A collaborative mindset with the ability to work independently.
