About the Role

As a Social Worker at CommonSpirit Health, you will play a vital role in providing social work interventions for priority patient and family needs. Our team is dedicated to delivering exceptional care and support to our patients, and we are seeking a skilled and compassionate professional to join our team.

Key Responsibilities
  • Provide social work services to patients and their families, including psychosocial assessment, domestic violence screening, child abuse and elder abuse interventions, and discharge planning for inpatients admitted to the assigned medical or surgical service.
  • Screen and intervene in cases of domestic violence, abuse and neglect, and substance abuse.
  • Collaborate with the interdisciplinary care team to develop and implement a comprehensive plan of care that addresses the patient's physical, emotional, and social needs.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and transition planning.
  • Document all interactions and interventions in accordance with departmental policy and regulatory requirements.
Requirements
  • Master's of Social Work from an accredited college or university and Licensed Independent Clinical Social Work (LICSW) license.
  • 2 years of Social Work experience in an inpatient setting.
  • Strong interpersonal skills, a working knowledge of individual and family dynamics, and excellent communication skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
