Key Responsibilities
  • Provide in-home primary care services to high-risk members, ensuring they receive high-quality follow-up care in a timely manner.
  • Develop and implement personalized care plans, taking into account each patient's unique needs and circumstances.
  • Collaborate with patients, families, and healthcare teams to educate and empower individuals to manage their health and well-being.
  • Conduct regular assessments, reviews, and evaluations to ensure patients are receiving optimal care and achieving desired outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices to provide evidence-based care.
  • Work closely with our Medical Assistant team to coordinate schedules, appointments, and patient communications.
Requirements
  • Master's degree from a four-year college and/or a professional certification beyond a four-year college.
  • Certificate of completion from a Master of Physician Assistant Studies program or an accredited Nurse Practitioner Program.
  • Current NCCPA or ANCC and/or AANP Certification required (Family Medicine, Internal Medicine, Adult Gerontology, Acute Care).
  • Unrestricted/Active CA PA or NP license.
  • Current CA DEA certificate required prior to start date.
  • 1+ years of experience as a practicing APC.
  • Active BLS certification.
  • EMR Proficient.
  • Excellent patient care and time management skills.
  • Ability to work independently and without direct supervision.
  • Ability to travel to multiple locations within designated regions.
Preferred Qualifications
  • 1+ years of experience in home health care or home visits programs.
  • Working knowledge of managed care, health plans, and medical groups.
  • Working knowledge of risk adjustment and coding.
