Medication Assistance Program Coordinator

Job Summary
As a Medication Assistance Program Coordinator at D-H Lebanon-MHMH, you will play a vital role in providing assistance to patients and their families in accessing medication assistance programs. This position requires excellent interpersonal and communication skills, as well as the ability to work closely with healthcare providers to assess patient eligibility and coordinate medication assistance applications and renewals.
Key Responsibilities
- Assess patient eligibility for medication assistance programs and provide information and support to patients and their families.
- Work closely with healthcare providers to coordinate medication assistance applications and renewals.
- Update information and resource directories, and maintain related literature and applications.
- Document services provided in patient medical records and applicable databases.
Qualifications
- Bachelor's degree in Social Work or Human Services field or equivalent education and experience.
- Excellent interpersonal, communication, and organizational skills.
- Required licensure/certifications: None.
