YOUR ROLE IN OUR SUCCESS:


A Pharmaceutical Care Specialist contributes to AHF by being assigned to multiple locations based on the scheduled needs of the Practice Sites, within the geographical area of the individual's primary residence.

This role expands the primary care functions of pharmacists, including personalized health promotion and disease prevention, administration of vaccines, long-acting HIV treatments, diabetes management, smoking cessation programs, and other relevant topics, in collaboration with other AHF Providers.


A Pharmaceutical Care Specialist enhances AHF's reputation by improving patient outcomes, expanding the services provided by AHF, creating opportunities for professional development and satisfaction, and supporting the ongoing evolution of the AHF Healthcare System.

The Pharmaceutical Care Specialist will work as part of a close-knit team to provide tailored health promotion and disease prevention, including immunization administration, diabetes management, smoking cessation programs, and other Subject Matter Expertise (SME) initiatives developed by AHF, with appropriate collaborative drug therapy management authority.

This role includes prescriptive authority where legally and organizationally permitted, such as PrEP and PEP therapies.

Licenses/Certifications:
State Pharmacist License in the state of application.
