Key Responsibilities:Scheduling:
  • Schedule and coordinate patient visits for home health and hospice care, ensuring optimal allocation of resources and staff availability.
  • Manage and adjust schedules to accommodate changes in patient needs and staff availability, maintaining accurate and up-to-date patient records within the system.
Coordination:
  • Communicate effectively with healthcare professionals, including nurses, therapists, and physicians, to ensure timely and accurate communication of schedule changes to staff and patients.
  • Collaborate with the clinical team to ensure patient care plans are followed, and troubleshoot any issues related to scheduling software.
Software Management:
  • Utilize WellSky/Kinnser software for scheduling and documentation, maintaining accurate and up-to-date patient records within the system.
Qualifications:Experience:
