About the Role

A Hug Away Healthcare, Inc. is seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team as a Case Manager in hospice care. This office-based position involves coordinating patient care, ensuring quality assurance standards, and providing administrative support.

Key Responsibilities
  • Quality Assurance: Ensure compliance with regulatory and accreditation standards, conduct regular audits of patient charts, identify areas for improvement, and implement corrective actions.
  • Administrative Support: Maintain accurate patient records, assist with onboarding and training, handle patient inquiries, collaborate with the billing department, and prepare documents for payor audits.
  • Communication: Serve as a liaison between patients, families, and healthcare providers, provide education and support, and communicate effectively with the healthcare team.
Qualifications
  • Current and valid LVN license in the state of employment.
  • Minimum of 2 years of experience in hospice and home health preferred.
  • Experience with quality assurance processes and regulatory compliance.
Skills and Abilities
  • Strong organizational and time-management skills.
  • Excellent communication and interpersonal skills.
  • Proficient in electronic medical records (EMR) systems and Microsoft Office Suite.
  • Ability to work independently and as part of a team.
  • Compassionate and patient-centered approach to care.
