- Liaison with care providers and treatment team members, ensuring seamless communication and coordination of care.
- Maintain accurate and up-to-date electronic health records, in compliance with regulatory requirements.
- Represent the organization in mental health support workgroups, contributing to the development of best practices and policies.
Requirements:
- Bachelor's degree in Social Work, Psychology, or a related field (preferred).
- Minimum of 2 years of experience working with individuals with serious mental illness and housing issues.
- Strong written, oral, and interpersonal communication skills, with the ability to build rapport with clients and colleagues.
- Proficiency in relevant software programs, with the ability to learn new systems and technologies.
- Empathy and a commitment to serving individuals with mental illness, with a passion for making a positive impact in their lives.
- Ability to work independently, manage a caseload, and meet deadlines, with a focus on delivering high-quality results.
