Key Responsibilities
  • Assess and Coordinate Care: Conduct thorough assessments of patients to identify their needs and develop personalized care plans. Coordinate with interdisciplinary teams to ensure timely and effective care transitions.
  • Develop and Implement Discharge Plans: Collaborate with patients, families, and healthcare providers to create comprehensive discharge plans that meet the unique needs of each individual.
  • Monitor and Evaluate Patient Progress: Regularly review patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Provide Education and Support: Educate patients and families on self-care, disease management, and community resources to promote healthy behaviors and independence.
  • Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure seamless care coordination and communication.

Requirements
  • Education: Bachelor's degree in Nursing or related field.

  • Certifications: Current RN license and certifications in care coordination or a related field (e.g., CCM, PRI).
  • Skills: Strong communication and interpersonal skills, with the ability to work effectively with diverse populations. Proficiency in electronic medical records and care coordination software.
