Nurse Care Coordinator

As a Nurse Care Coordinator at Saratoga Hospital, you will play a vital role in ensuring the health and well-being of our patients and employees. You will be responsible for assessing and interpreting data about patient status, identifying needs, and providing appropriate care. This includes physical, social, and emotional needs based on age and culture.
Key Responsibilities:
Assess and interpret patient data to identify needs and provide appropriate care
Identify and evaluate patient learning needs, abilities, and readiness to learn
Deliver patient care consistent with established standards of care, guidelines, policies, and regulatory standards
Obtain pre-placement health history and immunizations for prospective employees, volunteers, and students
Conduct pre-placement screening for new employees, including vital signs, vision test, color blindness test, urine drug screen, and lifting evaluation
Review employee laboratory results and provide follow-up care
Monitor cases requiring physician follow-up to ensure treatment consultation occurs
Research and provide recommendations for accommodating employees with handicaps
Review and investigate occupational injuries and illnesses
Provide counseling and follow-up care to ensure positive outcomes and communicate results with the EH team, PA, and Medical Director as needed
Communicate all occupational infectious disease findings to the manager of infection prevention
Coordinate blood-borne pathogen exposure program
Collect and interpret lab results, provide patient counseling and education related to disease process, and develop and revise plan of care for patient based on evaluation
Record OSHA reportable cases on OSHA Log 300
Organize and prioritize work assignments based on patient needs
Utilize critical thinking skills, respond to emergency situations per protocol, and demonstrate ability to make effective nursing judgment, including crisis and emergency situations
Review workers' compensation/disability claims and report all injuries to workers' compensation carrier
Coordinate transitional duty program for work-related employee injuries and communicate with Employee Health Team, Department Directors, and Workers Compensation Carrier
Participate in new employee orientation and provide training programs regarding occupational health and safety as requested
Participate in quality improvement activities, developing policies, evidence-based practice, and/or guidelines to improve patient care and assist with data collection and monitoring
Requirements:
Graduate of an accredited school of nursing
BSN preferred
