Job Summary

This position provides care to assigned residents in a caring, safe and efficient manner and is responsible for performing the following according to location standards, procedures and individualized resident care plans.

Key Responsibilities
  • Completes the assigned portions of the MDS for Nursing, ensuring accuracy and validity of the entire process.
  • Maintains residents' self-esteem, privacy, and confidentiality of personal information.
  • Assists in maintaining safe, pleasant, and orderly resident living areas.
  • Reviews and signs the MDS forms to signify completion of the process.
Requirements
  • Graduate of an accredited school of nursing.
  • RN nursing licensure for the state of practice is required.
  • Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 months of hire.
Work Environment

The noise level in the work environment is usually moderate. Frequency of travel may vary based on business need.

This position requires the ability to effectively communicate in English, both orally and in writing, and to maintain a stationary position or move in response to job tasks.
