Job Summary

St. Luke's Hospital Transitional Care Center is seeking a skilled MDS Coordinator to join our team. As a key member of our nursing staff, you will be responsible for ensuring the accurate and timely completion of Minimum Data Set (MDS) assessments, as well as maintaining a high level of patient care and satisfaction.

Key Responsibilities
  • Assessment and Documentation: Conduct thorough and accurate MDS assessments, including resident, family, and staff interviews, physical assessments, and medical record reviews.
  • Scheduling and Coordination: Maintain a schedule that meets both Medicare and OBRA requirements, ensuring all disciplines are informed of schedule changes.
  • Quality and Compliance: Ensure timely and accurate completion of MDS assessments, and submit them to the QIES system in a timely manner.
  • Care Planning: Develop and update resident comprehensive care plans, and conduct bedside assessments to identify patient needs.
  • Communication and Collaboration: Communicate discharge plans to the interdisciplinary team, and facilitate Medicare meetings as needed.
  • Additional Duties: Perform additional duties as assigned, including auditing skilled charts for completion of required documentation.
Requirements
  • Education and Experience: Proven experience as a Registered Nurse (RN) or similar role, with a successful completion of an RN license program.
  • Skills and Abilities: Excellent knowledge of medical and hospital terminology, excellent communication and interpersonal skills, and the ability to handle stress and work in a fast-paced environment.
  • Confidentiality and Integrity: Understanding of confidentiality obligations and nursing best practices, with a commitment to maintaining patient confidentiality and upholding the highest standards of integrity.
