Position Overview:
We are looking for a committed Registered Nurse to join our PACT program, which functions as a mobile treatment team. Elwyn has partnered with Fellowship Health Resources to enhance our Adult Behavioral Health Services, with the goal of delivering outstanding behavioral health care to individuals dealing with mental health challenges and substance use disorders.

Key Responsibilities:
  • Conduct detailed physical evaluations and collect health histories from clients.
  • Monitor both the physical and psychological status of clients, analyzing patient data to make informed clinical decisions.
  • Collaborate with physicians and other healthcare professionals, providing education to clients, families, and staff.
  • Comply with the Massachusetts Nurse Practice Act and Elwyn's policies and procedures.
Essential Duties:
  • Perform thorough nursing assessments upon admission and as health needs change.
  • Conduct regular assessments, including system reviews and risk evaluations.
  • Prepare discharge summaries to ensure continuity of care.
  • Present clinical findings to the Interdisciplinary Care Team (IDT) in a clear and organized manner.
  • Develop and regularly update healthcare plans for clients requiring ongoing nursing care.
  • Coordinate with various service teams to effectively communicate client health needs.
  • Monitor vital signs and health metrics as necessary.
  • Administer prescribed treatments and medications while ensuring client confidentiality.
  • Educate direct care staff on treatment administration and documentation.
  • Evaluate medication effectiveness and report any side effects to the Health Care Coordinator.
  • Assist clients in preparing for medical appointments, ensuring proper pre-appointment protocols are followed.
  • Document all relevant nursing activities and updates in the appropriate records.
  • Contribute to maintaining a safe and therapeutic environment by identifying potential hazards.
  • Provide nursing support across various units as required, demonstrating flexibility and teamwork.
Qualifications:
  • Must hold a valid Registered Nurse license in Massachusetts.
  • Minimum of one year of nursing experience, preferably in a developmental disabilities setting.
  • Proficient in using electronic medical record systems.
  • Able to thrive in a fast-paced environment while managing multiple priorities.
  • Exhibit strong judgment and problem-solving skills with minimal supervision.
  • Excellent communication and collaboration skills are essential.
  • Current CPR certification is required.
  • Must possess a valid driver's license and a clean driving record.
