Job Title: Client Care Coordinator

Job Summary:

The Client Care Coordinator will play a vital role in ensuring the delivery of exceptional client-centered services to all clients at Excellent Healthcare & PRP Services. This position will facilitate and ensure all referrals are assigned, contacted, and documented in a timely manner that reflects the company's mission and goals.

Key Responsibilities:

  • Ensure the delivery of client-centered services to all clients.
  • Deliver excellent client service to stakeholders.
  • Ensure continuous improvement of behavioral health treatment processes and operations.
  • Assist clients with scheduling appointments and coordinating services.
  • Collect and review referral information and consult with others as needed.
  • Work collaboratively with funding sources to obtain authorizations for reimbursement.
  • Follow up with clients who do not keep appointments and offer to reschedule.
  • Maintain documentation and statistics as required by the agency and funding sources.
  • Complete paperwork requirements and maintain quality, up-to-date clinical records.
  • Participate in meetings, staff development, and training as required.
  • Remain in compliance with internal and external policies, procedures, regulations, and standards.
  • Collect information from collateral resources to aid in determining diagnosis and need for treatment.
  • Model and support health and wellness activities and healthy coping skills for clients.
  • Transcribe medical dictation with absolute accuracy.
  • Demonstrate an understanding of medicolegal implications and responsibilities related to transcription of patient records.
  • Operate designated word processing, dictation, and transcription systems effectively and efficiently.

Requirements:

  • Minimum of an Associate Degree in Social Work, Psychology, or a related field from an accredited college or university.
  • At least one year of experience in a mental health setting preferred.
  • Accurate proofreading skills.
  • Computer skills.
  • Writing skills.
  • Ability to manage and organize information.
  • High motivation and flexibility.
  • Client-centered and organized approach to client care.
  • Good team player and confident person.
  • Demonstrate a high level of integrity, patience, compassion, care, and professionalism in dealing with pressured, unpredictable, and sensitive situations.
