Job Summary

The Medical Case Coordinator plays a vital role in ensuring seamless communication between patients and healthcare providers at Care Connect Medical. This position requires exceptional organizational skills, the ability to prioritize tasks, and excellent written and verbal communication skills.

Duties and Responsibilities
  • Generate and analyze daily reports to track patient progress and identify areas for improvement.
  • Review new patient referrals and treatment history to ensure accurate and timely scheduling.
  • Coordinate patient scheduling and communicate effectively with patients, providers, and colleagues.
  • Field incoming calls and transfer to applicable parties as needed, ensuring prompt and professional communication.
  • Monitor and report any errors or concerns within the CRM system.
  • Record all interactions in the CRM system to maintain accurate patient records.
  • Review and analyze new information or referrals submitted by providers to ensure timely and accurate processing.
  • Communicate with patients in a professional and empathetic manner, addressing their concerns and needs.
  • Escalate questions and concerns to the team lead in a timely manner, ensuring prompt resolution.
  • Monitor and assist with patient compliance issues, ensuring patients receive necessary support and resources.
  • Act as a liaison between medical providers and patients, facilitating effective communication and care coordination.
  • Ensure all work is conducted in compliance with HIPAA regulations.
  • Complete additional projects and tasks as assigned by management.
Required Skills and Abilities
  • Ability to greet patients and colleagues in a friendly and courteous manner.
  • Ability to resolve conflicts and address patient concerns in a professional and empathetic manner.
  • Ability to work independently and identify/solve problems in a timely and effective manner.
  • Outstanding problem-solving and organizational skills, with the ability to prioritize tasks and manage time effectively.
  • Excellent written and verbal communication skills, with the ability to communicate effectively with patients, providers, and colleagues.
  • Caring demeanor towards patients and colleagues, with the ability to provide empathetic and professional support.
  • Ability to multitask and manage multiple priorities in a fast-paced environment.
  • Ability to proofread and type at least 50 wpm, with proficiency in Microsoft Office Suite and similar software.
  • Basic understanding of office equipment and clerical procedures, including recordkeeping and filing.
Education and Experience
  • High School Diploma or GED required, with an Associate or Bachelor's degree preferred but not required.
  • 5 years of case management or medical administrative experience preferred, with experience in hospitality or customer service environments also preferred.
  • Bilingualism in English and Spanish is preferred but not required.
Physical Requirements
  • Prolonged periods of sitting at a desk and working on a computer, with occasional lifting of up to 15 pounds.
