Key Responsibilities:

  • Assess the psychosocial status of patients related to their illness and environment.
  • Carry out social evaluations and plan interventions based on evaluation findings.
  • Maintain clinical records on all patients referred to social work.
  • Provide information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
  • Provide information to patients or families/caregivers and community agencies.
  • Serve as liaison between patients or families/caregivers and community agencies.

Requirements:

  • Master's degree in Social Work from a program accredited by the Council on Social Work Education.
  • Hold a current and valid state license in Oregon.
  • Minimum of one year of experience in healthcare; home healthcare experience preferred.
  • Excellent verbal and written communication skills.
  • Strong organizational skills.
  • Current CPR certification with BLS.
  • Valid driver's license and auto insurance.
