Job Summary

We are seeking a skilled Certified Medication Aide to join our team at Fallbrook Rehabilitation and Care Center. As a Medication Aide, you will be responsible for providing direct patient care related to medication administration in a professional nursing home environment.

Key Responsibilities
  • Administer prescribed medications and treatments per residents' Physician orders in accordance with State regulations and Certification training.
  • Verify identity of resident receiving medication, drug, dosage, route, and time of administration as stated on the Medication Administration Record (MAR).
  • Monitor closely to observe ingestion, complete application, and reaction to medication given.
  • Document completion of tasks as rendered, any observed change of condition, reaction to drugs, significant incidents, and reasons med pass is not complete.
  • Report to supervising Charge Nurse immediately any refusals, COC, reactions, or errors.
  • Monitor, record, restock, and notify need to reorder medication inventories.
  • Maintain cleanliness and order of med carts, cabinets, and refrigerators by removing expired/discontinued/discharged meds with disposition forms per facility policy.
  • Communicate detailed report with interchanging Med Aides/Nurse about resident needs and observations for quality continuity of care.
  • Participate in meal service including setup, serving, clearing, and cleaning the dining room, assuring residents are positioned appropriately for meals, appropriate utensils are within reach of residents, assists with feeding as indicated.
  • Deliver and document all AM, PM, and Bedtime snacks, supplements, and hydration.
  • Provide support to Nurses/Practitioners/Physicians during treatments and examinations.
  • Assist fellow Aides by helping our residents with ADL's, transporting, answering call lights, and reporting observation of the resident to nursing supervisor.
  • Follow through with resident requests or needs, escalating to Charge Nurse as applicable.
  • Provide our residents with superior quality care using trained skills within certified scope of practice in accordance with facility policy.
Requirements
  • Applicants must have a valid and current Medication Aide Certification in the state of Texas.
  • Active CNA Certification is required.
  • BLS/CPR Certification is preferred.
  • Experience in a Long-Term Care / Skilled Nursing facility is preferred.
