Key Responsibilities- Orient individuals to programs, services, and community resources, providing them with the necessary information to make informed decisions about their care.
- Report clinical concerns, high-risk behaviors, and safety issues to clinical program staff and/or supervisor, ensuring a safe and supportive environment for all individuals.
- Meet with individuals to discuss disposition plans, personal safety plans, and create opportunities for them to address fears and concerns related to trauma and crisis.
- Document interactions and progress in Electronic Health Records, ensuring accurate and timely documentation.
- Work with program staff to develop and maintain resource directories, providing individuals with access to community and natural supports.
- Assist clinicians in developing and facilitating Skills Groups, promoting socialization and skill-building among individuals.
- Assist individuals in obtaining services that align with their life goals and recovery needs, including community and organizational supports.
- Arrange and provide transportation for appointments and services, ensuring individuals have access to necessary care.
- Promote collaboration and sharing of resources among individuals, addressing basic needs such as food, clothing, transportation, and housing.
- Help individuals identify their strengths, supports, and problem-solving techniques, empowering them to overcome fears and combat negative self-talk/self-stigma.
- Assist program staff in maintaining accurate documentation and attending program team meetings, trainings, and supervision as scheduled.
- Learn and maintain a trauma-informed environment of wellbeing, ensuring a safe and supportive space for all individuals.
- Perform other duties as assigned, contributing to the overall success of the team.
- High School diploma or G.E.D. equivalent for PS I, with no certification required.
- High School diploma or G.E.D. equivalent for PS II, with a Colorado Peer and Family Specialist Certification upon hire.
- Valid driver's license preferred, with a good driving record while employed.
- Lived experience with mental health issues, with a minimum of 1 year working experience in the mental health field and/or social work/human services field.
- Knowledge or ability to learn and practice trauma-informed principles and practices.
- Good communication skills, with the ability to work with diverse individuals from all walks of life.
- Ability to operate a computer and common software programs, with good time management skills and consistent attendance.
