Job Title: Peer Specialist I

The Colorado Coalition for the Homeless is seeking a Peer Specialist I to join our team. As a Peer Specialist, you will work collaboratively with people experiencing homelessness to facilitate a self-directed change process aimed at improving health and well-being.

Key Responsibilities:
  • Establish supportive relationships with peers to inspire hope and resilience.
  • Mentor peers regarding goal setting, skill-building, and problem-solving activities.
  • Provide individual and group peer counseling and support for peers in recovery from opioid use disorders.
  • Maintain confidentiality of all peer information.
  • Provide patient navigation and care coordination services in collaboration with healthcare staff and community partners.
  • Transport and/or escort peers to scheduled appointments.
  • Outreach to peers in the community to assess status and monitor needs.
  • Work cooperatively with property managers and residence staff to ensure proper services for peers.
  • Link peers with resources and supports within the community.
  • Serve as liaison with collaborating agencies to establish and maintain effective working relationships.
  • Facilitate group activities to promote social engagement and community connection.
Requirements:
  • Completion of an accredited Peer Specialist training or willingness and ability to complete training within first 6 months of employment.
  • Peer mentoring experience serving people experiencing behavioral health challenges and/or homelessness preferred.
  • Valid driver's license required.
  • Bi-lingual Spanish-English preferred.
