Job Summary

Molokai Ohana Health Care Inc is seeking a highly skilled and compassionate Family Nurse Practitioner to join our team of healthcare professionals. As a key member of our primary care team, you will provide comprehensive medical services to patients of all ages, working collaboratively with our Chief Medical Officer and other healthcare providers to ensure seamless care.


Key Responsibilities
  • Provide primary care services, including health promotion, injury and illness prevention, diagnosis, treatment, and monitoring of commonly encountered health problems and injuries.
  • Work collaboratively with our Behavioral Health and Dental providers to ensure coordinated care.
  • Perform testing, referrals to specialists as needed, write prescriptions, and perform office procedures within the scope of expertise, protocols, and available equipment.
  • Develop and implement patient-centered care plans, taking into account individual patient needs and preferences.
  • Stay up-to-date with the latest medical research, best practices, and guidelines to provide evidence-based care.

Requirements
  • Current Hawaii Advanced Practice Registered Nurse license to practice with prescriptive authority (APRN/APRN-Rx).
  • Masters in Nursing-Family Nurse Practitioner.
  • Current DEA license.
  • Current Hawaii NED Controlled Substance Registration Certificate (CSR).
  • Current Nurse Practitioner Board Certified.
  • Current BLS certified.
  • Knowledgeable and experienced with Electronic Medical Records.
