Job Overview

Crossroads Treatment Centers seeks a dedicated and compassionate Patient Recovery Coordinator to join our team. In this role, you will play a vital part in supporting patients' medical and personal recoveries from substance use disorder.

The estimated annual salary for this position is $65,000 - $70,000, depending on experience and location. Our Care Managers work within a localized geographical area and are expected to travel to multiple locations, with mileage reimbursement provided.

About the Role

A typical day as a Patient Recovery Coordinator may involve connecting and maintaining relationships with community partners for patient resources, working closely with a dedicated team to provide exceptional patient care, and participating in community awareness/engagement events to promote Crossroads and its mission.

You will be responsible for building individualized care plans, collaborating with healthcare professionals, and helping patients access necessary resources. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Requirements

To be successful in this role, you should have at least a High School Diploma or GED, with experience in healthcare, addiction medicine, community health, or care coordination preferred. A background in building individualized care plans and working with diverse populations is highly valued.
