We are seeking a talented Patient Assessment Coordinator to join our team

About the Role:

The Patient Assessment Coordinator will play a critical role in assessing the clinical appropriateness of clients referred to our organization for admissions. This role requires strong analytical skills, excellent communication skills, and the ability to maintain confidentiality.

Key Responsibilities:

  • Conduct thorough assessments of clients referred to our organization for admissions.
  • Document all relevant information and flag forms as needed.
  • Communicate effectively with physicians and family members of patients.
  • Develop and implement strategies to improve communication with referral sources.
  • Collaborate with other staff members to develop and maintain policies and procedures.
  • Participate in regular in-service meetings to stay up-to-date on best practices.

Qualifications:

  • High School diploma; Bachelor's degree, preferred.
  • Minimum two years of behavioral health experience or relevant work experience.
  • Excellent communication and customer service skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Strong analytical and problem-solving skills.
  • Proficiency in office software and equipment.

Salary Range: $62,000 - $82,000 per year, based on experience.
