POSITION SUMMARY

The Patient Care Partner-Medical Assistant (PCP-MA) plays a crucial role within the integrated care team at KCHC/Pillar Health. As a primary support to designated healthcare providers, the PCP-MA ensures that each patient visit is organized, centered around the patient and their family, and conducted in a safe, timely, and efficient manner. The PCP-MA collaborates within a care team co-managed by the Medical Provider and the Registered Nurse-Care Coordinator (RNCC) to effectively manage a patient panel through proactive care planning and coordination of necessary team members.

KEY RESPONSIBILITIES

The responsibilities outlined below are not exhaustive and may be adjusted as needed by management.

CLINIC ENVIRONMENT MAINTENANCE

· Execute clinic opening procedures daily to ensure readiness for patient care.

· Conduct exam room turnover between patients to maintain cleanliness and ensure availability of necessary equipment and supplies.

· Perform terminal cleaning and restocking at the end of the day based on patient volume.

· Uphold general facility cleanliness and integrity in line with care standards.

· Implement infection control measures, including hand hygiene and sterilization protocols.

· Assist in maintaining supply levels and reorder supplies as necessary.

· Ensure clinical equipment is functional and maintained according to manufacturer guidelines.

PRE-VISIT PREPARATION

· Review daily schedules and patient records to identify necessary screenings and issues to address during visits.

· Ensure all required patient information is available prior to the provider's visit.

PATIENT FLOW MANAGEMENT

· Facilitate patient visits to maintain an efficient schedule and optimize care delivery.

· Welcome patients and their families, ensuring privacy and comfort during their visit.

· Conduct comprehensive patient rooming processes, including documentation of vital signs and other relevant data.

· Complete medication reconciliation and manage refills as needed.

· Follow up on overdue lab tests as per policy.

· Monitor provider voicemail and return calls to patients, addressing concerns or escalating as necessary.

MEDICAL ASSISTING DURING VISITS

· Support medical providers during examinations, treatments, and procedures, utilizing appropriate techniques.

· Document patient interactions accurately in compliance with standards.

· Provide patients with visit summaries and reinforce follow-up instructions.

· Schedule future appointments as directed by the provider.

MEDICATION ADMINISTRATION

· Administer injections and immunizations according to established protocols.

· Document all medication administrations accurately.

· Ensure immunization records are updated in the appropriate registry.

DIAGNOSTIC TESTING

· Assist with and perform diagnostic tests as required.

· Collect specimens for laboratory procedures and prepare them for shipping.

· Conduct approved point-of-care tests and participate in compliance monitoring.

TEAMWORK AND COMMUNICATION

· Engage in daily huddles to coordinate activities and resources.

· Communicate any scheduling delays and collaborate with the team to address them.

ADDITIONAL RESPONSIBILITIES

· Participate in quality improvement initiatives and maintain patient confidentiality.

· Understand medical coding and keep accurate medical records.

POSITION REQUIREMENTS

Education and Experience

· Graduate from an accredited Medical Assistant Program

· Must provide official diploma and transcripts.

· Obtain Certified Medical Assistant status within six months of hire.

· CPR Certification required.

· Proficiency in Spanish (oral and written) preferred.

Physical Requirements

· Must be current on all immunizations.

· Ability to stand and walk for the majority of the workday.

WORK SCHEDULE

Availability to work up to 40 hours during clinic hours, Monday to Thursday, and rotating Saturdays.
