Case Manager, Homecare

RiseBoro Community Partnership is seeking a skilled Case Aide to join our team. As a Case Aide, you will play a vital role in ensuring that our clients receive the necessary services to maintain their eligibility for homecare and Assisted Living Program (ALP) services.
Key Responsibilities- Develop and implement plans to ensure clients receive necessary services
- Communicate with staff members, clients, family, and/or caregivers to ensure services are established
- Maintain accurate and detailed client data in designated databases
- Provide information on services, benefits, and entitlements to clients
- Provide comprehensive case management and coordination services, including intake assessment, benefit assessment, goal setting, and referrals
- Participate in education of healthcare workers and advocate on behalf of clients
- High School diploma or equivalent required; Associate degree or 4+ years of experience in healthcare, homecare, or human services preferred
- Bilingual English/Spanish required
- Mental Health First Aid skills-based training course completion a plus
- Competitive salary range: $24-31/hourly DOE
