About the Role

RiseBoro Community Partnership is seeking a skilled Case Aide to join our team. As a Case Aide, you will play a vital role in ensuring that our clients receive the necessary services to maintain their eligibility for homecare and Assisted Living Program (ALP) services.

Key Responsibilities
  • Develop and implement plans to ensure clients receive necessary services
  • Communicate with staff members, clients, family, and/or caregivers to ensure services are established
  • Maintain accurate and detailed client data in designated databases
  • Provide information on services, benefits, and entitlements to clients
  • Provide comprehensive case management and coordination services, including intake assessment, benefit assessment, goal setting, and referrals
  • Participate in education of healthcare workers and advocate on behalf of clients
Requirements
  • High School diploma or equivalent required; Associate degree or 4+ years of experience in healthcare, homecare, or human services preferred
  • Bilingual English/Spanish required
  • Mental Health First Aid skills-based training course completion a plus
