About the Role

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build Meaningful Relationships - Establish and maintain strong relationships with residents and their families, providing personalized care and support.
  • Provide High-Quality Care - Deliver exceptional care and services to a consistent group of residents, meeting their physical, emotional, and social needs.
  • Develop and Implement Care Plans - Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each resident.
  • Partner with the Care Team - Work closely with the care team to ensure seamless delivery of care and services, promoting a positive and supportive environment for residents.
Requirements
  • Dedication to Senior Care - A genuine passion for providing high-quality care and services to seniors.
  • Excellent Communication Skills - Strong verbal and written communication skills, with the ability to build rapport with residents, families, and colleagues.
  • Organizational and Time Management Skills - Ability to prioritize tasks, manage time effectively, and maintain accurate records.
  • Previous Experience in Senior Care - Previous experience working with seniors, preferably in an assisted living or dementia care setting.
